Subject: [PATCH 44/78] net: qualcomm: Add BAM DMUX Ethernet/IP driver (v2)

BAM DMUX is used as network interface to the modem on some older
Qualcomm SoCs like MSM8916 or MSM8974.

v2:
 - Switch to Raw IP mode
 - Set parent device and device type
 - Expose channel ID in "dev_port" attribute
 - Always open/close channel with network interface
 - Set default MTU to 1500
 - Add IFF_NOARP/IFF_POINTTOPOINT flags
 - Use netif_receive_skb() instead of netif_rx_ni()
 - Disable noisy DEBUG prints
